Access medical records online
You can access your inpatient medical information online through our patient portal.
Baptist Health protects your healthcare information in compliance with federal and state requirements. If you would like a paper copy of your medical records, please download the Release of Information form for your hospital listed below, then print, complete, sign and mail (or fax) the form to the hospital.

When you come to pick up your medical records, please bring a photo ID. If someone other than the patient is picking up the records, that person needs to bring a photo ID and an original signed authorization letter from the patient. There may be a fee for creating a copy of your records.

Don’t have a MyChart account? Orlando Health offers patients a secure, fully online records request tool that verifies your identity by asking for a photo of your Photo ID which can be taken via webcam or smartphone. There is no charge for this service.
